Reflections on Patient Safety, Communication, and Ethical Practice in Nursing

Abstract
This paper reflects on a critical incident involving a medication error and communication breakdown in an emergency department (ED) setting, highlighting the ethical, clinical, and leadership lessons learned. As an assistant nurse manager (ANM), I explore how this event emphasized the need for effective communication, ongoing education, and active supervision, particularly for new graduate nurses (GNs) navigating high-pressure environments. Drawing from hospital protocols, the American Nurses Association (ANA) Code of Ethics, and a biblical worldview, the discussion focuses on strategies to enhance patient safety through education, simulation exercises, and fostering a collaborative work culture. By addressing these key areas, nurse leaders can create environments that prioritize patient welfare, minimize errors, and uphold the highest standards of ethical practice and compassionate care.
